Combat: extract shared attack-resolution primitives
Pull effectiveAttackBonus, attackCritFloor, attackConnects, and the post-hit player damage stack out of combat_engine.go into combat_primitives.go so the upcoming turn-based engine resolves an attack identically to auto-resolve.
